Registered Nurse (RN)-Case Manager

The Health Resource Manager is responsible for providing case management services for SVMC sub acute patients.
Requirements: Graduation from an accredited school of nursing and a Bachelor’s of Science of Nursing preferred; applicants holding a valid RN license and currently matriculated into a BSN program may be considered. Prior experience in utilization review, discharge planning and/or case management preferred. 3-5 years of recent clinical experience required. VT Nursing License required by date of employment. Case Management Certification required within 2 years of employment
The case manager is responsible for coordinating a patient-centered plan of care to meet individual needs by:
- Completing intake interviews to identify a holistic plan of care; facilitating multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to discharge
- Developing a patient transition plan, providing patient/caregiver education and proactively advocating for patient needs and goals across the interdisciplinary team to ensure an approach that is sensitive to each specific patient and family’s needs
- Assisting providers in determining patient’s level of care in accordance with InterQual criteria
- Actively seeking out collaboration with providers to improve overall quality and efficiency of care
Graduation from an accredited school of nursing and a Bachelors of Science of Nursing required. Five years of clinical nursing experience and BS required. BSN preferred. Prior experience in utilization review, discharge planning and/or case management preferred. VT Nursing License required by date of employment. Case Management Certification required within 2 years of employment.
