Position: Experienced Home Health Occupational Therapist
Location: Prince George’s and Montgomery Counties
Employment Type: Full-time, Part-time, PRN
Position Overview:
We are seeking an experienced and compassionate Home Health Occupational Therapist to join our team. The ideal candidate will be dedicated to enhancing the quality of life for patients through personalized, evidence-based occupational therapy services in the comfort of their homes. This position offers the opportunity to work independently while collaborating with an interdisciplinary team to ensure the highest standard of care.
Responsibilities:

Conduct comprehensive assessments of patients’ physical and functional abilities.

Develop and implement individualized treatment plans based on patient needs and physician recommendations.

Provide skilled occupational therapy interventions to improve mobility, strength, balance, and daily function.

Educate patients and caregivers on exercises, safety strategies, and preventive care.

Monitor progress, reassess goals, and adjust treatment plans for optimal outcomes.

Document clinical findings, interventions, and patient responses accurately and on time.

Collaborate with physicians, nurses, and healthcare team members to coordinate care.

Ensure compliance with HIPAA, Medicare, and other regulatory standards.

Supervise Certified Occupational Therapy Assistants (COTAs).

Participate in professional development activities to enhance clinical skills and maintain best practices.
Qualifications:

Graduate of an accredited occupational therapy program with a valid Maryland license.

Minimum 3 years of clinical experience (home health or skilled nursing preferred).

Strong skills in assessment, treatment planning, and therapeutic interventions.

Ability to recognize and respond to medical emergencies.

Excellent interpersonal and communication skills for patient, caregiver, and team collaboration.

Proficiency with EMR systems (Kinnser WellSky or Homecare Homebase preferred).

Reliable transportation and valid driver’s license required.
