Position Overview:
We are seeking an experienced Home Health Occupational Therapist to join our dynamic team. The ideal candidate will be dedicated to improving the quality of life for our patients by providing expert occupational therapy services tailored to individual needs. This role offers the opportunity to work independently while collaborating with interdisciplinary teams to ensure the highest standard of care.
Responsibilities:

Conduct comprehensive assessments of patients' physical and functional abilities.

Develop and implement personalized treatment plans based on physician recommendations and patient goals.

Provide skilled occupational therapy interventions to improve mobility, strength, balance, and overall function.

Educate patients and caregivers on therapeutic exercises, safety, and injury prevention strategies.

Monitor patient progress and adjust care plans to optimize outcomes.

Document all evaluations, interventions, and patient responses accurately and timely.

Collaborate with physicians, nurses, and other healthcare professionals for coordinated care.

Ensure compliance with regulatory standards, including HIPAA and Medicare guidelines.

Participate in ongoing professional development to enhance clinical knowledge.
Qualifications:

Graduate of an accredited occupational therapy program with a current Pennsylvania license.

Minimum of 3 years of clinical experience, preferably in home health or skilled nursing.

Strong clinical reasoning, assessment, and treatment planning skills.

Ability to recognize medical emergencies and respond appropriately.

Excellent interpersonal and communication skills.

Proficiency in electronic medical records (Kinnser WellSky or Homecare Homebase preferred).

Reliable transportation and valid driver’s license required.

Experience supervising COTAs preferred.
