LCSW or LPC-Play Therapist

The LCSW/LPC-Play Therapist completes assessments, conducts individual, group and family therapy, coordinates care with psychiatrists, referral sources, primary care physicians, assists with completion of all necessary paperwork and documentation in the Electronic Medical Record.
Minimum Qualifications:
- Master’s degree from an accredited school of social work/or counseling
- Eligible for licensure in the state of Louisiana
- Five or more years of post-master’s experience working with very young children, adolescents, and their families, along with three years supervisory experience preferred.
- Infant Mental Health Certification, or willing to obtain.
- Experienced in using Evidence Based Practices for very young children.
- Proficient in use of EMR/Computer experience
- Prior outpatient treatment experience
- High level of ethics in marinating patient confidentiality and advocacy
- Be able to utilize telephone, computer terminal, printer, fax, and copier while performing certain duties
Physical and Mental Requirements:
- Evaluates and helps resolve urgent or emergent situations
- Stands, walks, sits and converses most of the day
- Carries relatively light materials
- Good listening skills, alert, problems solving skills under stressful conditions
- Creative in use of therapeutic techniques
- Good memory, concentration, judgment, patience and initiative
- Communicates effectively with patients, physicians, and other staff in written and verbal form
Essential Duties/Responsibilities
Clinical Responsibilities:
- Complete initial screening/assessment
- Complete initial treatment plan within collaborative partners as needed.
- Provide individual, group, play and family therapy to clients and their families.
- Provide crisis intervention
- Evaluate client’s progress and report to referral physician
- Document groups, individual sessions, and any other significant contact with the client within the week facilitated
- Complete discharge summary at end of treatment
- Document any referrals
- Audit charts for completeness and quality
- Manages the Athena inbox to make sure tasks are being completed
- Coordinates personnel and services
- Assists staff (PLs) with therapy related issues/questions
- Assists in the development of plans with IMH team to operationalize strategic initiatives.
Case Management Responsibilities:
- Identifies and arranges resources needed to meet the individual needs identified and coordinates with community services and support.
- Take all necessary actions in assuring the implementation of comprehensive service plans including serving as advocate for the client to enable him/her to receive needed services, identification of program needs, service, gaps, and recommending strategies
- To maintain ongoing coordination of services and support system to ensure continuity of intervention
- To maintain client records, review and update client evaluations, complete report of services provided, services needed, and all other clinical or administrative records deemed necessary
- Meet with families, staff of various agencies and programs, develop strategies for maintaining client in his/her community setting
- Ensures participation in the BHQIP as directed and ensures data/information is collected and reported in a timely way, within the overall AHL QIP and used for improving processes and outcomes. Participates in audits as assigned.
- Understands and supports UDS (BH) required data collections and outcomes.
Documentation:
- Maintains proper documentation of all assessments, individual, group or family therapy
- Completes all documentation requirements in a timely manner
- Comply with all policies and laws regarding the release of information
Other Responsibilities:
- Assist in obtaining referral from Medicaid Primary Care Physicians (PCP)
- Monitor sessions and referral expiration dates
- Cooperate and assist in facilitating billing of services
- Follow job safety requirements.
- Report incidents or accidents within 24 hours of occurrence to immediate supervisor
