Nashua Center of Nashua, NH is growing! We are seeking a dedicated and experienced Manager to lead our team in delivering high-quality services in the Human Services field. QUALIFICATIONS:
• Bachelor’s degree in Human Services, Social Work, Psychology, or a related field.
• Minimum of 2 years of experience in Human Services or a related area.
• Experience with American Sign Language (ASL) is preferred.

RESPONSIBILITIES & ILLUSTRATIVE DUTIES :
• Assist and train in home care Providers to ensure that all programs meet all certification requirements.
• Attend pre-Individual Service Agreement (ISA) meetings to discuss potential goals and issues.
• Attend annual ISA meetings, complete all required documentation prior to the meetings and upon receipt of completed ISA.
• Communicate needs of individuals to appropriate support and partnered agencies, i.e. Area Agencies, other vendors, physicians, etc.
• Be able to physically assist individuals' with all lifting and transferring needs. Ability to lift 30 lbs.
• Visit individuals in their homes regularly at varied times (minimum bimonthly, including evening and weekend visits when necessary).
• Complete Quality Assurance visit checklist and/or additional documentation for visit. Follow up on all quality assurance concerns and considerations to ensure effective, quality programming.
• Coordinate and attend meetings between day program and individual providers. Attend required team meetings.
• Serve as a resource for troubleshooting safety concerns or potential adaptive needs within provider homes.
• Ensure cross training and consistency on programming when clients receive both residential and Day Services from Nashua Center.
• Assure attendance reports, progress notes and related documentation are completed accurately and on time.
• Attend in-service trainings that are appropriate for professional development.
• Must be able to assist with personal care as needed or in crisis situations.
• Complete requirements for Medication Administration.
• Develop and foster an attitude of team effort and cooperation throughout the programs and with the individual's families/guardians.
• Advocate for individuals and program needs.
• Assist in developing and monitoring the implementation and documentation of all programs.
• Promote the principle and practice of Social Role Valorization.
• Maintain and enforce the rules of safety outlined in the Employee Safety Program.
• Other duties as assigned.
