What You’ll Do
-
Conduct thorough nursing assessments in patient homes, evaluating their full spectrum of health and support needs
-
Complete initial OASIS assessments and other required visits such as recertification and discharge, and collaborate with LPNs for ongoing care

Develop and update personalized care plans in partnership with the care team and families
-
Deliver skilled care including wound care, medication management, IV therapy, catheter care, and more
-
Monitor patient progress and adjust care as needed while keeping the care team informed
-
Provide education to patients and caregivers to empower them in their journey to wellness
-
Coordinate with therapists, aides, social workers, and community resources to ensure well-rounded support
-
Keep clear, compliant, and timely documentation in our electronic health record system

Offer emotional support and guidance to patients and their families during difficult times

Follow infection control and safety protocols to ensure well-being for all involved

Take part in case reviews, care planning, and performance improvement efforts
What You Bring
-
Current RN license in Virginia (LPN experience may also be considered)
-
2 to 3 years of nursing experience, ideally in home health or community care
-
Strong clinical judgment and proficiency with skilled nursing tasks
-
Excellent communication and empathy to connect with diverse patients and families
-
Familiarity with EHR systems and confident documentation skills
-
Experience in wound care and diagnosis strongly preferred

Valid driver’s license and reliable transportation to travel locally

A passion for providing compassionate, high-quality, and patient-centered care
