Service Areas: Fairfax, Loudoun, and Prince William Counties, VA

Key Responsibilities:
-
Conduct comprehensive speech therapy assessments in patients' homes.

Develop and implement individualized treatment plans based on patient goals and physician directives.

Deliver skilled interventions to improve communication, swallowing, and overall speech/language function.

Educate patients and caregivers on techniques, strategies, and safety measures.
-
Monitor patient progress and make necessary adjustments to treatment plans.

Document clinical findings and interventions accurately and in a timely manner.

Collaborate closely with nurses, physicians, and other healthcare providers to ensure coordinated care.

Maintain compliance with HIPAA, Medicare, and agency standards.

Participate in continuing education to remain current with best practices.
Qualifications:
-
Graduate of an accredited speech-language pathology program.
-
Active license to practice in the state of Virginia.
-
Minimum of 2 years of clinical experience preferred, ideally in home health or skilled nursing settings.

Strong assessment, planning, and treatment skills for adult and geriatric populations.

Ability to recognize and respond to medical emergencies appropriately.
-
Excellent communication and collaboration skills.

Experience with EMR systems; Kinnser/WellSky or Homecare Homebase preferred.

Familiarity with OASIS documentation preferred.
-
Reliable transportation and valid driver’s license required.
