Summary
Florida Health Care Plans, Inc. is seeking a full-time LCSW or an LMHC with 3 years’ experience to join our Behavioral Health Team in Edgewater. The ideal candidate will have experience in dual diagnosis of substance abuse and mental illness, group, individual, family therapy & case management. Must be comfortable with children, adolescents, and adults.
Duties and Responsibilities
• Case list will consist primarily of substance abuse patients and dually diagnosed
• Facilitating groups within the department focused on substance abuse, relapse prevention and mental health stability.
• Evaluation, assessment, and treatment of dually diagnosed patients.
• Promotes positive patient relations
• Completes required documentation in a timely and legible manner consistent with appropriate documentation standards.
• Coordinates with clinical staff (crisis team, nursing, psychiatry, therapy) in assisting patients and their families in understanding, accepting, and following medical recommendations
• Performs independently and manages other duties as assigned.
Qualifications
• Computer skills and experience with Microsoft Office programs
• Verbal and written skills necessary to effectively communicate with various members of the health care team, other health facilities, community health related organizations, various external parties, and regulatory agencies
• Master’s degree in social work or other Human Services Field from an accredited university
• Florida Licensed Clinical Social worker (LCSW), Mental Health Counseling (LMHC)
• Experience in working with patients with substance use disorders.
• 3 years of relevant work experience preferred
