JOB SUMMARY
The Physical Therapist is a licensed independent contracted professional who provides comfort physical therapy services for hospice patients.
JOB SPECIFICATIONS
- Must be licensed in the state of operation with one year experience in physical therapy, and provide a copy of the license to ACH.
- Must provide proof or copy of:
- Current CPR card
- Automobile insurance
- Valid Driver's License
- TB testing
- Hepatitis B vaccination or waiver
- Background check
- Medical malpractice insurance
- Must have demonstrated knowledge of and well developed skills in treatments to relieve pain and develop or restore function, using physical means such as exercise.
- Demonstrated knowledge of hospice services, principles of care, and rules and regulations governing hospice services.
- Must have valid driver's license and reliable transportation.
- Ability to develop and maintain positive working relationships with staff, community partners, and referral sources.
- Ability to communicate effectively verbally and in writing.
ESSENTIAL FUNCTIONS
- Assist the physician in the evaluation of Patients, including the application of muscle, nerve, joint, and functional ability tests.
- Treat patients to relieve pain and develop or restore function, using physical means such as exercise. Evaluates and observes pain completely with all assigned patient, including pain scale (intensity) and quality (pain character, frequency, location, and duration) appropriate to the patient's age.
- Direct and aid patients in therapeutic exercises, muscle re-education, gait and functional training, and activities of daily living, including transfer activities and prosthetic training.
- Observe, record, and report to the physician and RN Case Manager the patient's reaction to treatment and any changes in patient condition.
- Instruct patients in the care and use of medical equipment, and prosthetic and orthotic devices.
- Instruct health care personnel and family members in certain phases of physical therapy, when appropriate. May give input into the evaluation of the care provided by Home Health Aides, and provide in-services for Hospice staff.
- Submit written clinical reports and assessments using Auburn Crest Hospice (ACH) forms, unless otherwise approved, within 24 hours of the visit to the patient.
- Submit an invoice to ACH for professional services rendered monthly with appropriate patient summary and other appropriate forms
- Participate in Interdisciplinary Team (IDT) meetings or provide a patient status report to the RN Case Manager.
- Document all communication with other ACH workers and physician on his/her notes or on conference notes.
- Gives adequate notice when terminating service to the patient and provides a discharge OASIS or summary and plan.
- Therapist is responsible to annually calibrate and check all personal equipment used in the physical therapy treatment of ACH's patients, and to provide a copy of all documentation regarding calibration, equipment checks, or any necessary repairs on an annual basis, or upon request.
- Schedule visits with the patient as outlined in the Plan of Care and frequency as determined by the IDT.
ADDITIONAL EXAMPLES OF WORK PERFORMED
- Perform or accompany Nursing staff for patient consultations as needed or directed.
- Performs related duties as assigned or required.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
- Duties are primarily performed in office or patient residence environments and require sufficient personal mobility and physical capability to permit functioning in these environments.
- Must be able to lift up to twenty (20) pounds.
- May be required to work irregular hours including evening and weekend availability.
- Local travel required. Occasional travel outside of area may also be required.
