POSITION TITLE: Community Health Worker
REPORTS TO: Community Health Worker Supervisor
SUMMARY: The individual will conduct in-person and telephone conversations with individuals, providing prevention education, general health and wellness education, and community engagement. The individual will also identify available healthcare and social resources to community members.
DUTIES AND RESPONSIBILITIES
- Responsible for implementing efforts, including health-focused events and collaboration with the CDPH to improve service access.
- Support vaccination drives, training, and Medicaid redetermination efforts,
- Conduct pre-event canvassing in targeted communities by walking door to door and disseminating information in public areas to community member
- Provide resource navigation services and offer onsite support, including bilingual translation services.
- Adheres to the mission of Central States SER
- Coordinating resource navigation efforts with Supervisors and the Chicago Department of Health.
- Conducting interviews in a professional manner that reflects emotional and cultural awareness
- Collect and record accurate information using Customer Relationship Management (CRM) software such as UniteUS and Microsoft Teams.
- Interviewing individuals and utilizing assessments make recommendations of resources available, along with disseminating health information
- Providing contacts with regulated information on isolation and quarantine procedures as directed
- Collaborating with designated local authorities in supporting public health initiatives
- Facilitating health checks, as well as referring contacts to available community resources
- Participate and conduct in person community outreach events to educate and inform individuals on various topics related to public health.
- Complying with all Health Department regulations, including those aimed at protecting personal information, along with HIPPA compliance
- Protect and maintain individuals’ privacy and confidentiality
- Provide service to individuals in communities with low life expectancy and economic hardships.
- Performs other related duties as assigned

JOB REQUIREMENTS:
- Must be able to work in the office and remotely in the served communities
- High School Diploma or GED is required but College-level education is preferred
- Reside in a Priority Community Area as identified by The Chicago Department of Public Health such as North or South Lawndale, Westside of Chicago
- Proficiency in utilizing the Windows Operating system and performing data entry; previous experience in CRM software is an additional benefit
- Advanced proficiency in English is required; bilingual Spanish proficiency is required
- Proficiency in MS Word, Excel, and Outlook Express, as well as data entry
- Ability to comply with regulations and communicate official information in an appropriate manner
- Advanced ability to coordinate efforts, facilitate intervention, and maintain records
- Exceptional organizational skills and sound judgment
- Excellent interpersonal skills to assist distressed individuals from different cultural backgrounds
