Position Summary
-
The Cardiogenic Shock Coordinator is a registered nurse who manages acutely ill patients from admission through discharge with the focused purpose of maximizing clinical outcomes through the early recognition of impending shock, initiation of hemodynamic support and decreasing variations in care. Utilizing strong clinical, leadership, communication and interpersonal skills, he/she works corroboratively with the care team of nursing, physicians, surgeons, respiratory care and pharmacists to provide leadership, oversight, management and coordination of the evidence-based plan of care for recognition, management, interventions and mechanical circulatory support in cardiogenic shock patients.

Core Job Functions

Identify patients experiencing Cardiogenic Shock (CS) through review of clinical panel, history and clinical presentation. Facilitate compliance with CS guidelines and outcome measures of CS program.

Provides clinical, professional expertise in development of the plan of care for patients with CS. Analyzes data regarding compliance with outcome measures, standardize treatment plans and care bundles.

Implements approved protocols (including medications and treatment plans) to assure compliance with CS guidelines and best practices. Collaborates with physicians to assure collaboration between cardiology, pulmonology and cardiac surgery to improve continuity of care. Rounds daily to evaluate appropriateness of orders, treatment plans, nursing interventions, physician documentation to provide a standardized and comprehensive approach to the management of CS. Facilitates compliance with outcome measures including LOS reduction and reduction in mortality.

Communicates progress with the CS initiatives through an interdisciplinary team and quality meetings. Through analysis of data, reports identified patterns of care, benchmark best practices and opportunities for improvement.
