JOB SUMMARY

The Discharge Coordinator/PCT serves as the facilitator of the patient throughput/discharge process and is a valued member of the healthcare team. They collaborate with case managers and other members of the healthcare team to implement the discharge plan agreed upon by the RN, case manager, physician, patient/chose support system, and other involved members of the interdisciplinary team. Responsible for performing assigned patient care activities and assisting licensed nursing personnel. The role is designed to support a wide variety of operational support functions that support discharge-planning activities.
