We are seeking a Nurse Navigator to coordinate care for high-risk, chronically ill patients across the continuum, and to promote effective education, self-management support and timely healthcare delivery to achieve optimal quality and financial outcomes.
REQUIREMENTS: Graduate of an accredited school of nursing, Bachelor's Degree preferred. Current RN license in the State of Massachusetts. Three to five years of clinical-related job experience, case management experience preferred. Non-smokers, skills in healthy lifestyle promotion desired.
