Position Purpose:
Disease Navigators act as care coordinators, facilitating communication between the high-risk patients with chronic disease, their families, caregivers, providers and other members of the healthcare program. Outreach to patients for 7-day follow-up appointments from the ER and who may qualify for different chronic condition programs with the completion of timely Health Risk Assessments. The main focus is to offer individualized assistance to patients, families and caregivers, in order to help overcome health care system and community barriers and help facilitate consistent and timely medical care, from the initial hospitalization, through all phases of the Disease Management process.

Education and Work Experience:
- High school diploma or GED
- Experience with or ability to learn assessment tools and interventions for hypertension, diabetes and mild to moderate depression
- Experience working with underserved, transient populations preferred
- Experience working with patients who have co-occurring mental health, substance abuse, and physical health problems preferred
- Experience with screening for common mental health and/or substance use disorders preferred
Knowledge and Skills:

Registry (per diem) Caregivers are caregivers who work less than 1,000 hours in a calendar year. Registry caregivers are employees of Sinai Health System but are typically on an as needed basis. These caregivers may be called upon to cover full-time or part-time caregivers during PTO events or sick leave as well as to provide supplemental coverage. Registry Caregivers are not eligible for Sinai’s benefit programs with the exception for Sick-C, PPO Medical Plan, Employee Assistance Program (EAP) and non-match eligible contribution to the Sinai Health System Consolidated 403(b) retirement savings plan.

1) This is a Hybrid position. (3 days on site and 2 days' work from home). This can change at any time due to the department needs. 2) Complete monthly Health Risk assessments on new members assigned to the Medical Group. 3) Outreach to members who are low risk and have been identified as having an ER visit and/or Observation Stay 4) Assist with scheduling 7-day follow-up appointments with PCP and/or Specialists after ER/Observation Stays 5) Provide reminder phone calls for various schedule appointments 6) Contact patients who have been identified with chronic conditions to complete the applicable assessments 7) Assist patient with any SDOH issues that may be identified 8) Assist patient with adherence to existing self-management goals or development of goals 9) Present outcomes at weekly huddles for patients who have been in the ER and/or Observation Stay 10) Main timely and appropriate documentation on patient integrations, in the approve disease management record/system 11) Develop and maintain excellent working knowledge of diseases assigned and weeks information on current/future state for navigation processes and techniques 12) Develop effective working relationships with PCPs, Specialist, care team members, Administrative Leadership and Physician Champion 13) Must be proficient in Excel 14) Must have excellent computer skills
