EARLY CHILDHOOD CASE MANAGER

GENERAL SUMMARY/BASIC PURPOSE OF JOB:
The case manager will be responsible for completing registration forms for SNAP customers. Responsible for creating a tracing form to identify registered customers and facilitate orientations. Facilitate client follow-up and assist customer in the resource center. The Case Manager meets individually with the participant to analyze results, make referrals, and assist with career path selection. The Case Manager is responsible for accurate case notes. The Case Manager will track all supportive service activities. The Case Manager will facilitate workshops and participate in outreach activities. The Case Manager will monitor client to ensure that each client reach their 30, 60 and 90 day benchmarks. The Case Manager will submit documentation of clients' status to Program Manager on a timely basis. Participation is required in program training sessions.
MINIMUM EDUCATION:
Bachelor's degree in social work, sociology, psychology or related field preferred.
MINIMUM WORK EXPERIENCE:
Minimum four (4) years of experience in the area of social services with a minimum two (2) years as a case manager. Minimum one (1) year working in a training and employment setting.
SKILLS & ABILITIES:
Excellent oral and written communication skills. Demonstrates ability to work independently.
Basic computer skills required.
