Home To Stay is a leading private pay home health agency serving South Jersey communities. With a focus on exceptional client care across Burlington, Camden, Gloucester & Salem counties, we're seeking a compassionate RN to join our dedicated team.
Core Responsibilities:
• Lead and conduct comprehensive home health aide skills assessments, ensuring our caregiving team maintains the highest standards of care delivery
• Perform detailed client evaluations encompassing physical health, psychological well-being, social needs, and environmental safety factors to develop holistic care plans
• Design and implement personalized care strategies that maximize client independence while ensuring safety and quality of life
• Collaborate with families, caregivers, and healthcare professionals to coordinate optimal care delivery
• Mentor and support home health aides to enhance their skills and ensure excellent care standards
• Provide in person and in office skills assessments of Home Health Aides
Clinical Impact:
• Drive quality improvements in home care services through evidence-based assessments and care planning
• Build trusted relationships with clients and families to ensure continuity of care
• Contribute to team development through sharing expertise and best practices
Required Qualifications:
• Active New Jersey RN license in good standing
• Graduate of an accredited nursing program (BSN preferred)
• Demonstrated experience in geriatric care
• 5+ years of relevant nursing experience preferred (home care agency and/or geriatrics)
• Valid driver's license and reliable personal transportation with current insurance
• Exceptional assessment and clinical decision-making abilities
