What You'll Do:
· Provide individual therapy and support in clients' homes, schools, or the community
· Help clients access vital services and stay on top of benefits
· Create and lead engaging therapy groups
· Keep detailed records and stay on top of clinical documentation
· Be a steady, supportive presence in your clients’ lives

Essential Responsibilities, include but are not limited to:
- Conduct Mental Health Assessments and Semi-annual treatment planning for assigned clients using the State approved IM/CANS tool ensuring the treatment plans incorporate objectives that reflect the identified needs of the individuals served.
- Provide individual therapy, individual assessment and treatment planning to clients ages ranging from 18-65 years and older clients.
- Clinicians provide therapy services to individuals: in the community, at EU day centers and via telehealth.
- Provide information to outside agencies as requested to coordinate and assure the appropriateness of services.
- In conjunction with the client’s treatment team, keep the family, guardian and other Envision Unlimited staff informed of the client’s progress.
