Job Description/Summary
The Licensed Social Worker for Seniors is familiar with the needs of older adults and how these needs impact mental health. This professional takes a trauma informed approach to care and exhibits an understanding of evidenced based practice. The provider plays an important role in the expansion and enhancement of services at PBHG by providing direct care to older adults and building relationships with partners in the community who support the same population and their families.
Responsibilities
- Provides comprehensive Psychosocial Assessments to gather symptoms and treatment history, assess mental status, diagnosis current mental health symptoms, and determine the most appropriate level of behavioral health care.
- Provides individual and group counseling to older adults and their families.
- Provides in home/in office support to frail and vulnerable older adults to assist with all social determinants that impact mental wellness including finances/retirement, medical issues, housing advocacy, and linkages to all services provided by Ocean County Office of Senior Services.
- Provides case management as needed including timely and effective advocacy, consultation, service coordination, and phone contact to community resources and related professionals. Documentation reflects all contacts made with and/or on behalf of each client.
- Develops community relationships with local senior communities and arranges In-community groups to increase socialization and foster community connection.
- Provides Senior Workshops and community mental health events throughout the county to provide education on mental health issues and skills to promote senior wellness.
- Collaborates with multidisciplinary treatment team to identify clients that are in need of community-based services and engages these vulnerable adults.
- Records level of service quarterly into government issued software program.
- Participates in performance improvement activities as assigned.
- Completes all documentation within required timeframes based on policy.
- Engages in regular supervision with Director of Adult Outpatient, and attends treatment team meetings, including agency-wide clinical case reviews.
Competencies
- Motivated, Independent Professional.
- Organized with excellent communication skills.
- Bilingual in Spanish is preferred.
Qualifications
- Education: Master’s degree in social work or mental health counseling.
- Licensure: LSW or LAC required
- Experience: Experience working with older adults.
