About myPlace Health
myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.
Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.
myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.
About This Role:
If you’re someone who leads with empathy, finds purpose in caring for others, and loves helping teammates grow, the Lead Personal Care Attendant (Lead PCA) role might be your perfect fit.
As a Lead PCA, you’ll be at the heart of our care team—delivering high-quality, person-centered support to older adults in our clinic, day center, and home settings. In addition to assisting with daily living activities, you’ll serve as a mentor to fellow PCAs, offering guidance, encouragement, and hands-on training to ensure every participant receives the compassionate care they deserve.
You’ll collaborate closely with the Interdisciplinary Team (IDT) and PCA Supervisor to help coordinate care, maintain smooth daily operations, and create a safe, supportive environment for all. This is a hands-on leadership role where your voice matters, your presence makes a difference, and your dedication to medically complex and frail older adults helps shape their daily experience.
Whether you're helping someone start their day with dignity or guiding a new PCA through best practices, your impact will be deeply felt—every single day.
What Does Success Look Like As A Lead PCA?
- Be a Care Champion: Provide compassionate, respectful assistance with daily living activities like escorting, toileting, bathing, grooming, laundry, and meal support—whether in the clinic, day center, or even in the participant’s home.
- Lead by Example: Model personal care excellence, professionalism, and warm participant engagement for your PCA teammates.
- Support & Mentor: Help onboard new team members and offer hands-on training and real-time guidance to ensure they feel confident and supported.
- Keep Us Running Smoothly: Assist in coordinating daily assignments and staffing coverage in collaboration with the PCA Supervisor, Nursing leadership, or Center Manager.
- Stay Observant: Keep an eye on participants’ well-being—physically and emotionally—and promptly communicate any changes to licensed staff or the Interdisciplinary Team (IDT).
- Travel Buddy: Assist participants in and out of vehicles and serve as a medical escort for offsite appointments when needed.
- Mealtime Matters: Work with our food service team to make sure participants receive the right meals and provide assistance during feeding as needed.
- Supply Super Sleuth: Monitor personal care supplies and let us know when it’s time to restock essentials like incontinence products or grooming items.
- Champion Clean & Safe Spaces: Help maintain tidy, sanitary, and welcoming care environments by following all infection control practices.
- Keep Us Connected: Complete basic documentation and communication logs to ensure everyone stays in the loop—especially when concerns need escalation.
- Bring the Joy: Support social programs, mobility exercises, and activities that uplift the daily experience for our participants.
- Team Player Vibes: Participate in daily huddles and share updates on participant care needs or observations.
- Safety First (and Always): Respond calmly and effectively in emergencies, assist with drills, and support participants during evacuations or transfers when needed.
- Care Collaborator: Work hand-in-hand with our amazing team of nurses, social workers, therapists, and others to deliver truly person-centered care.
- Pitch In Where It’s Needed: Step up to support additional responsibilities as needed to ensure smooth operations and exceptional participant care—because teamwork makes the dream work!
What Does An Ideal Candidate Look Like?
- Heart First: Deeply committed to serving high-risk seniors and frail older adults with empathy, dignity, and respect
- Hands-On Experience: Proven background as a Personal Care Attendant (PCA), Certified Nursing Assistant (CNA), or Home Health Aide (HHA)
- Elder Care Comfort: Confident and compassionate when working with elderly and medically complex individuals
- Peer Supporter: A natural team player who enjoys mentoring others and contributing to a positive, collaborative care environment
- Adaptable Spirit: Flexible and able to support participants across settings—including the clinic, day center, and home—depending on need
- Certified & Prepared: CPR and First Aid certified (or willing to obtain within 60 days of hire)
- Qualified to Care: Current CNA or HHA certification required
- Language Matters: Bilingual abilities are always valued and warmly welcomed
- On the Move: Valid California driver’s license required
