GENERAL:

The MS Coordinator provides clinical nursing and administrative leadership and oversite for med-surg services, case management/discharge plannining and utilization review at HSH promoting an interactive positive culture. Working collaboratively with medical staff, administration and nursing services, the coordinator develops collegial relationships and allocates available resources to ensure efficient, effective quality care consistent with standards of care, policies, and procedures and organizational goals. This position has 24-hour accountability for all Med-Surg operations. The MS Coordinator is responsible for creating safe, healthy environments that support the work of the health care team and contribute to patient engagement. The role is influential in creating a professional environment and fostering a culture where interdisciplinary team members are able to contribute to optimal patient outcomes and grow professionally.
Accountable for the clinical practice of nursing. Coordinate and implement standardized policies, procedures, and programs that support the consistent and reliable clinical practice of nursing adhering to the Wyoming State Board of Nursing and Wyoming State Practice Act requirements and compliance with governmental agency law and regulations. Champion efforts that promote the best practices, lessen practice variation, reduce waste, and ensure the highest quality experience and clinical outcome for every patient.
Assess the current state of recruitment and retention of clinical staff and implement initiatives that will enhance the organization’s ability to successfully recruit and retain top talent throughout nursing services; and expand training/educational opportunities that will foster new employee on-boarding, career growth and professional satisfaction. Ensure employees are qualified and properly trained to perform assigned job duties.
Assess current staffing models and performance to national benchmark productivity measures/targets. Create new innovative staffing models that will improve overall efficiencies, quality care, nursing productivity, patient and employee satisfaction and support recruitment and retention strategies.
Demonstrate financial acumen and resource management in establishing department budgets, prioritized capital requests, identifying revenue opportunities, completing variance reports ensuring accurate charge capture activities.
Champion and mentor an exceptional patient experience using evidence-based methods.
Engage staff to understand and embrace their individual accountability in achieving facility and department quality, safety, service and financial goals. Actively lead in the assessment and monitoring of daily/weekly/monthly performance and achievement for established goals.
Build collaborative relationships with providers and other departments. Coordinates appropriate medical staff committees. Identify areas in need of change, establish and implement a positive and proactive approach to systematic change.
Be a visible leader, engaging front line staff, supporting facility teams, addressing high and low performance in a timely manner, engaging in constructive dialog, managing up decisions.
Drive a culture of quality and patient safety. Collaborate with providers and staff on the use of evidence-based protocols with measureable results. Actively participate in quality improvement activities and reporting. Lead rapid cycle improvement initiatives.
Actively engage in improving patient flow, throughput, hand-offs, efficiencies, and communication between departments.
Assist in the preparation for state, federal, regulatory and accrediting surveys including continuous readiness.

CASE MANAGEMENT/DISCHARGE PLANNING
POSITION SUMMARY: The case manager/discharge planner is a registered nurse who is responsible for supervision of the nursing team, ensuring patient-centered care coordination, discharge planning, patient advocacy and assessment and planning.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Working with patients, families, healthcare providers and outside entities to ensure a smooth and effective healthcare journey.
Developing comprehensive plans for patients leaving the hospital, including arranging transportation, referrals and home health services.
Supporting patients and families in accessing resources and navigating the healthcare system.
Evaluating patient needs, developing care plans, and coordinating services.
UTILIZATION REVIEW
POSITION SUMMARY: The utilization review nurse is a registered nurse who is responsible for medical necessity review, resource utilization, adherence to guidelines and claim review.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Evaluating patient care to determine if it meets established medical necessity criteria.
Ensuring healthcare resources are used appropriately and efficiently to minimize costs.
Following established utilization review guidelines and protocols.
Reviewing medical records to justify hospital stays or other services to insurance companies.
SUPERVISORY RESPONSIBILITIES:
Directly supervises med-surg employees and is responsible for case management/discharge planning and utilization review. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

EDUCATION and/or EXPERIENCE
Bachelor’s Degree in Nursing preferred or ability to attain same within five years. Five years of MS nursing experience, including two years in a supervisory or leadership capacity preferred.
Current Wyoming Registered Nurse license
Familiar with Wyoming State Nurse Practice Act
LANGUAGE SKILLS:
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write policies and procedures that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors.
MATHEMATICAL SKILLS:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and is knowledgeable in metric, apothecary and household measurements and can convert from one system to another.
REASONING ABILITY:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to modify care based on the developmental / functional age of the patient as well as that of the patient's ethnic background.
CERTIFICATES, LICENSES, REGISTRATIONS:
- BLS required
- ACLS within one year of hire
- PALS within one year of hire
- NRP within one year of hire
OTHER SKILLS AND ABILITIES:
Ability to act as a patient advocate. Ability to interact with patients, families, visitors and co-workers. Ability to interact assertively and tactfully when dealing with conflict and in group solving activities. Ability to demonstrate a professional, open minded approach in identifying problems and resolving problems/conflicts.
Age Specific:
Must be able to provide care according to the developmental level and or chronological patient served. This includes, but is not limited to, identifying the needs and abilities of each patient, and demonstrating an understanding of how a patient’s chronological and developmental age influences his/her need for adaptations related to his/her abilities. Also, an understanding of how a patient’s age impacts his/her ability to deal with and adapt to the care needed/provided is required. General census of neonatal to geriatric patients.
