Essential Functions:
- Manages primary caseload of Hospice patients per physician's instructions and in compliance with regulations
- Must do on-call rotation
- Travel to Applewood Village and Pines Village
- Implements and updates care plans to address patient's nursing needs
- Educates and supports patients, family, and staff on nursing care and patient needs
- Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements
- Supervises Licensed Practical Nurses and Hospice Aides when needed
- Some on-call responsibilities with additional financial compensation
Qualifications:
- Active and valid NJ RN license
- Preferred certification: Certified Hospice and Palliative Nurse (CHPN)
- Preferred minimum of 2 years experience in hospice or palliative care but willing to train the right person
- Ability to collaborate within an interdisciplinary setting
- Knowledge and understanding of hospice philosophy and principles of death/dying
- Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements

- Robust critical thinking and problem-solving abilities
- Capable of working independently and collaboratively in a multidisciplinary team
- Proficient in electronic medical record systems and relevant software applications
