Position Summary:
We are seeking a compassionate and detail oriented LPN Care Coordinator to join the IHC team. Your primary responsibility will be calling patients regarding their medications. Medication calls will focus on ensuring patients have the correct prescriptions, understand how to take them, and address any barriers to adherence. In addition, you will make transition of care calls. This role plays a crucial part supporting patients after an emergency department visit, hospital stay, or other healthcare transitions. You will be part of a multidisciplinary team including a Physician, Social Worker, Registered Nurses and a Pharmacist.

Qualifications:
- Current, unrestricted Licensed Practical Nurse (LPN) license in the state of Ohio
- Minimum of 3 to 5 year’s experience in discharge planning, care coordination or case management role. Previous experience in provider office would be beneficial.
- Training in motivational interviewing or health coaching techniques
- Current CPR training certificate
Skills:
- Strong Communication and patient education skills, the ability to explain medical information in a clear and empathetic manner
- Ability to work independently while collaborating with a multidisciplinary team
- Knowledge of chronic disease management, medication reconciliation, and healthcare navigation
- Knowledge of community resources and support programs available to patients
- Problem solving skills and the ability to assess and address social determinants of health that may impact patient recovery or medication adherence
Responsibilities & Expectations:
- Review medication lists with patients, ensuring accuracy and understanding of dosage, frequency, and purpose of each medication
- Identification of barriers to medication adherence
- Education on the importance of taking medications as prescribed and not skipping doses unless director by a doctor
- Follow up and reinforcement of instructions
- Conduct post discharge follow-up calls to patients after hospital stays, emergency department visits, or other healthcare transitions
- Provide education and resources to patients and caregivers regarding condition management, follow-up appointments, and available support services
- Documentation of patient interactions in electronic medical record or other medical management system
- Coordinate with primary care providers, specialists, and other healthcare professionals to address any patient concerns or gaps in care
- Seeks supervisory guidance/approval as appropriate.
- Portrays professional image: follows dress code, communicates with internal and external customers in a professional manner, including appropriate verbal and written grammar.
- Promotes and demonstrates professional standards to enhance the development of the department.

Working Conditions:
- Hours of operation are 7:00 AM to 5:00 PM, Monday through Friday, of four hours shifts will be scheduled five days per week. Occasional adjustment of work hours may be necessary to meet deadlines.
- Some travel may be required for which you will be required to use your own automobile with mileage reimbursement as applicable.
- Office environment with moderate noise level or company’s designated office space including hybrid work hours
- Lunch and break periods must be coordinated to maintain adequate office staffing during departmental operating hours.
- Subject to frequent interruptions and changes in priority of duties throughout the day.
- Works with highly confidential data, information and situations.
Category II-Includes tasks that involve no exposure to blood, body fluids, or tissues, but employment may require performing unplanned Category I tasks. The normal work routine involves no exposure to blood, body fluids, or tissues by exposure or potential exposure may be required as a condition of employment. Appropriate protective measures will be readily available to every employee engaged in Category II tasks.
