Reports to the Vice President for Heart, Vascular and Stroke Service Line at the system level with a dotted line to Director for Cardiovascular Services or designee at each hospital across the system. Collaborates and consults with physicians, Structural Heart Coordinators, nursing care team and other healthcare providers in the coordination of care delivered to the Vascular patient populations. Is an integral member within Heart, Vascular and Stroke co-management care networks. Works closely with the Leadership team on identifying opportunities for improvement in reducing care variation across the system at each entity level related to vascular care. Recommends corrective actions and provides ongoing monitoring of key initiatives.

Works closely with Vice President of Heart, Vascular and Stroke Service Line for the BHS to:
- Promote and drive standardization of care throughout the health system at different entity locations to reduce care variation using evidence-based medicine (SVS and VQI clinical guideline standards).
- Attends and regularly reports data to Heart, Vascular and Stroke service line co-management councils and committees in regard to performance improvement initiatives (metrics).
- Provides expertise to lead, manage and support in guiding strategic initiatives.
- Assists in developing key clinical indicators for routine monitoring of performance statistics for internal use and communicates performance both internally and externally.
- Demonstrates optimal healthcare quality and value by leading teams in implementation efforts, as well as instructing and mentoring others to guide effective care and optimal patient outcomes.
- Focus of accountabilities will encompass patient care across the continuum (acute and post-acute settings).
- Serves on various PI projects or teams to improve patient quality and outcomes.
- Utilizes clinical data to develop and support best practice guidelines.
Works with Vascular Team for Program Development:
- Develop referral coordination through ED and PCPs
- Develop Central referral process that allows for triage to the right physician based on expertise and best location for care.
- Develop clinical pathways for vascular patients
- Develop protocols based on SVS and VQI best practice.
- Develop PI metrics based on VQI scorecard.
- Implement monthly Vascular Case Conference with EGH and MHSB physicians to review cases and films.
- Integrate with stroke and neurology program.
- Initiates surveillance plan for patients after interventions and for patients without an intervention.
Internal/External Data Analysis and Clinical Database Management:
- Works with essential departments and individuals for data collection, tracking and reporting outcomes supporting strategic planning goals.
- Develops and maintains processes for data validation and integrity for selected metrics and initiatives.
- Benchmarks with other hospitals and health systems to identify best practices and processes for performance improvement initiatives, and coordinates selected external benchmarking projects with State/Federal and others when indicated.
- Gives feedback to physicians and other clinical staff on failed core measure cases to assure future compliance with core measures.
- Responds to specific data requests from physicians, administration, and staff for assigned core measures and for other topics as needed.
- Handles data quality reviews which may involve interpretation and decision making as to appropriate course of action.
- Actively participates in the development of clinical databases for internal/external data analysis.
Performs other functions to maintain personal competence and contribute to the overall effectiveness of the health system by:
- Completing other job-related assignments and special projects as directed.
- Adheres to established productivity standards.
- Networks to maintain positive working relationships as appropriate with key customers (physicians, surgical services, office managers, therapists, case managers, DME coordinators, nurses, diagnostic staff and other specialty care team members).

- Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience
- The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a Bachelor of Science Degree in Nursing. Previous clinical navigation or project coordination and management will compliment this position. Experience in leading teams to drive changes within the system through development and patient safety improvement projects. Additionally, a minimum of two years� experience in the clinical setting of vascular area is also preferred. A valid and current RN license in the state of Indiana and BLS certification are required.
Knowledge & Skills
- Requires thorough knowledge of clinical care practices, procedures to meet targeted patient populations (SVS and VQI clinical guidelines and Joint Commission core measures).
- Working knowledge of computer hardware and software including desktop applications, Clinical Data Management applications and tools (Microsoft products such as: Excel, WORD and PowerPoint).
- Demonstrates effective analytical and problem-solving skills.
- Experience with quality management systems such as Lean/Six Sigma and other performance improvement tools.
- Requires working knowledge of research methodology.
- Demonstrates clear, effective communication skills, including verbal, written, and listening skills.

Working Conditions
- Works in various environments with frequent changes in job demands.
- Requires travel to referral sources, satellite and other locations.
- Work hours will fluctuate based upon the needs of the patients served.
