Organizes and administers individualized, professional patient care to patients of a primary care practice. Provides nursing care as directed by the provider and in accordance with the objectives, policies and procedures of the hospital and the state Nurse Practice Act. Provides clinical leadership to other members of the care team and creates a caring and healing environment where patients and their family are the center of the care process.
Primary Job Responsibilities:

Patient Assessment: During patient visit, assesses patient’s physical/psycho-social status, reviews medications and confirms medication reconciliation as appropriate. Obtains vital signs, height and weight. Provides results of assessment to provider and documents findings in the patient’s medical record. Analyzes relevant data and integrates assessment results into care planning, goals and outcomes.
Patient Care: Collaborates with the provider to formulate and implement a plan of care as appropriate based on assessment and diagnosis. Refers patients to case manager/social worker to ensure access to additional services and resources. Assures consistency in standards of care and the provision of care meets all applicable joint commission standards to ensure patient safety. Triages calls from patients, family members and community health providers. Engages in relationship based care to create a caring and healing environment. Advocates for patients and families.
Communication: Applies patient specific communication skills in interactions that are culturally sensitive and population/age specific. Language barriers and sensory and cognitive deficits are accommodated as needed. Communicates information to the provider that is pertinent, timely and concise. Manages conflict among other team members in a direct and effective manner.
Patient Education: Provides information to patient/family with consideration to individual needs. Assesses for barriers to learning and motivation. Consults with other members of the care team to enhance patient and family education. Participates in developing patient education materials. Reviews patient educational materials with patients and their families. Provides patients with appropriate teaching materials resource booklets.
Clinical Leadership: Delegates and/or assigns care that is consistent with role, scope of practice and abilities of other clinical care staff (LPNs, medical assistants, patient care coordinators). Actively participates in problem solving. Provides feedback to other members of the care team to enhance their education and knowledge. Coordinates care across the team. Conducts research as needed to respond to patient questions and concerns. Uses critical thinking skills to analyze data to support clinical decision making. Displays an ability to solve problems, reason and learn utilizing evidenced based practice. Assures staff practice within scope of practice.
Documentation: Ensures all documentation is accurate and in compliance with institutional standards.
Additional Responsibilities
Additional Duties: Performs additional duties as assigned by the provider and office/practice manager to provide an efficient and pleasant atmosphere for patients.
Minimum Qualifications:
Education, Training & Experience
- Graduate of an accredited school of nursing
- BSN preferred.
Certifications / Licensure
- Licensed to practice in New York State

Expected Salary Range: $31.65-$52.37/hr
