We are currently seeking a dedicated and experienced Case Manager to join our Camp Meade team in Linthicum, MD and support patients through comprehensive case management and discharge planning.
Position Summary:
As a Case Manager, you will play a key role in supporting the continuum of care for patients by assessing needs, developing care plans, coordinating services, and collaborating closely with both the clinical team and patients" families. Your work ensures that every patient receives the right support at the right time—empowering their recovery and long-term success.
What You"ll Do:
- Implement and manage individualized treatment and discharge plans for patients.
- Meet regularly with patients, families, and interdisciplinary teams to coordinate care and communicate progress.
- Educate patients and families on available resources and aftercare options.
- Maintain accurate and timely documentation in compliance with regulatory standards.
- Collaborate with providers across behavioral health, medical, and community resources to ensure comprehensive support.
- Identify and connect patients with additional resources, including legal aid, housing, and vocational programs.
- Participate in recreational and therapeutic activities as part of patient engagement.
- Supervise family visits and liaise with legal representatives, caseworkers, and probation officers when necessary.
What We"re Looking For:
- Bachelor's degree in Social Work, Counseling, or a related Human Services field.
- 1–2 years of experience in a behavioral health setting.
- Knowledge of addiction, detox processes, co-occurring disorders, DSM criteria, and behavioral health terminology.
- Excellent communication and documentation skills; ability to clearly explain treatment plans and next steps.
- Strong organizational skills and ability to manage multiple cases effectively.
- Proficiency in Microsoft Word, Excel, and Outlook.
- Ability to work collaboratively with clinical teams and community partners.
Preferred Qualifications:
- Experience with Electronic Medical Records (EMR).
- Bilingual in English and [Insert Language if Applicable].
- Familiarity with legal case coordination for patients in treatment.
