We're seeking a compassionate and detail-oriented Per Diem Registered Nurse (RN) to join our team in providing high-quality, client-centered care at our community mental health clinic. The RN will play a key role in supporting psychiatric care by conducting brief medical screenings, performing medication follow-ups, and collaborating with interdisciplinary team members to support client recovery and wellness.
Duties and Responsibilities:
- Conduct brief medical screenings and assessments of new and returning clients to identify health concerns and support psychiatric evaluation.
- Perform follow-up assessments for clients on psychiatric medications, including monitoring for side effects, effectiveness, and adherence.
- Coordinate care with psychiatrists, nurse practitioners, therapists, and case managers.
- Document all client interactions and assessments accurately and promptly in the Electronic Health Record (EHR).
- Track and document medication compliance, flagging concerns for the prescriber and treatment team as needed.
- Monitor for adverse drug reactions, drug-drug interactions, and changes in client condition that may warrant medication adjustments.
- Educate clients and caregivers on psychiatric medications, including purpose, potential side effects, proper usage, and the importance of adherence.
- Collaborate closely with psychiatrists and prescribing providers to communicate medication-related observations and participate in care planning.
- Support crisis intervention efforts within the RN's scope of practice as needed.
- Maintain compliance with clinic policies, licensing regulations, and HIPAA standards.
- Prepare and administer injectable medications per clinical protocols and safety standards.
- Coordinate laboratory testing by submitting lab requests and reviewing results to support clinical decision-making.
- Participate in interdisciplinary case reviews and team meetings, as requested.
Qualifications:
- Current and valid RN license in the state of New York.
- Minimum of 1 year of experience in a psychiatric, community health, or primary care setting preferred.
- Knowledge of psychiatric medications and common side effects.
- Strong assessment and documentation skills.
- Ability to work independently and collaboratively with a multidisciplinary team.
- CPR/BLS certification required.
- Bilingual Spanish is highly preferred
Compensation is based on credentials and experience.
Working Conditions:
The primary work location varies. This is a professional work environment where a computer, desk, chair, and office supplies will be provided in an office.
Physical Requirements:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the role above. While performing the duties of this job, the employee is regularly required to hear and speak on the phone or through virtual telehealth sessions conducted on a computer throughout the day, stand and sit for an extended period, and be able to write and answer emails in addition to carrying objects no greater than 20 pounds, when needed.
