Under the direction of the Manager Care Coordinator, the Sr Licensed Social Worker is responsible for providing leadership and resources to the Care Management Department. As the Sr Licensed Social Worker, you will assist the Supervisor with ensuring adequate staffing per each hospital unit, mentoring/preceptorship for new social workers, serving as a lead for psychiatric placement dispositioning, and functioning as an expert resource. You’ll provide staff education on motivational interviewing and substance abuse interventions by using a variety of treatment modalities to assist patients and families in alleviating or resolving social, financial, cultural, and emotional problems and barriers related to the patients' successful management of issues related to illness, health care, and rehabilitation. This position provides psychosocial assessment, discharge planning, coordination, community referrals, and support/counseling on various assigned units.
Annual Salary Range: $71,032.00 - $104,769.60
Qualifications
Education:
- Requires a Master’s Degree in Social Work.
Experience:
- Requires a minimum of four (4) years of experience providing social work services in an inpatient or skilled facility health setting.
- Prefer prior supervisory experience.
Certification/Licensure:
- Must be licensed by the Arizona Board of Behavioral Health Examiners as a Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- Ambulatory Care Management: Must have a valid fingerprint clearance card issued by the Arizona Department of Public Safety or submit a completed Affidavit at the time of hire. Must complete fingerprint clearance card application within seven (7) days of hire date. Must present actual card within ninety (90) days of applying for a fingerprint clearance card.
- Must obtain a BLS card through the American Heart Association (AHA) training center within 30 days of hire.
Knowledge, Skills, and Abilities:
- Requires the ability to read, write, and speak effectively in English, including professional-level usage or grammar, spelling, and punctuation.
- Must know about chemical dependency and mental illness.
- Must have a working knowledge of case management, hospital, and complex psychosocial interventions, and be well versed in community resource management.
- The position requires excellent communication and clinical skills and professional rapport with physicians.
- Requires excellent organizational skills, working with professionals, and maintaining confidentiality standards.
- Critical thinking ability is essential.
- Excellent organizational skills and the ability to handle multiple tasks simultaneously are required.
- Bilingual and bicultural are a plus.
