JOB SUMMARY
Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospice and Carroll Hospital Center. Provides professional, comprehensive, family-oriented services to individuals in the home setting. Assesses patients' and families' psychosocial, environmental, and financial needs. Formulates, implements, and evaluates a plan of care in collaboration with patient, family, and other caregivers, and provides case management as appropriate. Offers counseling services to provide information and support during all facets of the dying and grieving process to help the hospice patient/family cope with terminal illness. Coordinates with community and agency homecare referrals for services.
REQUIREMENTS
- Required Master's Degree Successful completion and graduation from an accredited school of social work, with a Masters Degree in Social Work.
- 2-4 years Requires minimum of three years of clinical experience; one year in a community health setting preferred. Required
- LCSW - Certified Social Worker
SPECIFIC REQUIREMENTS
Fleet Safety Requirements. Interpersonal communication (both oral and written) required in order to educate and comfort patients and their families; maintain effective contacts. Maintain appropriate documentation records. Professional licensure required in Maryland and Pennsylvania
