The role of the Nurse Care Coordinator is to work & support PSV’s Pulmonary Clinic, coordinate care for Pediatric Pulmonary patients, serving as the clinic liaison. This role is 80% Administrative and 20% Clinical. This role will be working Part-Time day shift, 20 hours a week, in Fairfax, VA.
Job Description
Administrative:
- Coordinates care for the outpatient pulmonary population in collaboration with the physician and healthcare team.
- Facilitates all aspects of patient care from diagnosis and throughout the continuum of treatment, including disease specific education, symptom management, and referrals to support.
- Interacts with patients during physician office visits and via phone, assessing patient needs. Triage’s patient phone calls and requests appropriately to healthcare team.
- Documents all interactions, care provided and findings in the medical record.
- Utilizes order entry for diagnostic testing, prescription refills and other patient instructions requiring provider signature.
- Follow up on insurance authorizations.
- Acts as a liaison with other departments and institutions to clarify questions related to the care of the patient such as orders and medical records.
- Provides additional support to the providers including tracking results of laboratory, radiological and pathological studies, triaging results and follow up with patients as needed with results and instructions.
- Serves as a clinical resource for staff, patients/ caregivers, and other team members.
- Coordinates with clinic staff to ensure a positive patient flow, waiting room, and exam room experience.
- Communicates patient concerns and needs to the healthcare provider.
- Provides timely responses to patient inquiries and timely completion of assigned tasks.
- Adheres to and employs appropriate sterile techniques and universal precautions guidelines.
- Other duties as assigned.
Clinical:
- Assist families become comfortable with biologic administration by performing in clinic biologic administration, teach and assess return demonstration of biologic administration teaching.
- Successfully provides disease process and treatment education to patient and family.
- Assist support staff and providers in clinic PRN with medication administration, discharge, and teaching.
Minimum Qualifications Required
- Graduation from an accredited school of nursing
- Valid licensure in the state of Virginia as a Registered Nurse (RN) or ability to obtain license in Virginia
- One (1) year of Pediatric experience
- Five (5) years of RN work experience
- Valid BLS/CPR certification
