JOB SUMMARY
Is responsible for medication needs of inpatients and outpatients. This includes dispensing activities as well as reviewing order for appropriateness and compliance with hospital guidelines and restrictions. Communicates with and makes recommendations to other hospital staff. Executes clinical programs such as writing TPN orders, pharmacokinetics, intravenous to oral conversion, providing drug information, and other performance improvement activities to promote quality and safety in the medication use process. Ensures that the pharmaceutical care provided meets the unique needs of patients with regard to age and development. Provides clinical pharmacy services pertaining to the neonate (0-28 days), infant (28 days-1year), pediatric (1-12 years), adolescent (13-18 years), adult and geriatric patient, ensuring that the pharmaceutical care provided meets the unique needs of the patient with regard to age and development.
REQUIREMENTS
- Doctorate Degree PharmD - Required
- Previous Pharmacy experience - Required
- Board Cert Pharmacotherapy Specialist - Preferred
- Cert Anticoag Care Provider - Preferred
- RX - Pharmacist License - Required within 90 days
- BLS - AHA Healthcare Provider - Basic Life Support - Required
