JOB RELATIONSHIPS

Responsible to Case Manager as appropriate & Team Leaders and Clinical Manager. Provides patient care as directed by Case Managers. Consults with and keeps Case Managers informed of activities, needs, incidents, complaints or problems related to delivery of quality patient care.

DEFINITION OF POSITION
-
The Home Health Staff Nurse is responsible for: The continuous improvement in assessing, developing, and delivering a well-organized plan of care as it relates to nursing services and case management.

HHA supervision, patient care, monitoring, and communication with other appropriate team members and clinical manager.
-
Working within the scope of the Nurse Practice Act regarding judgment, nursing skills, education, and experience.

QUALIFICATIONS
-
Professional
-
Current license to practice as an RN in the state of North Carolina.
-
Knowledge of current trends and techniques in nursing appropriate to practice setting.
-
One year experience working as an RN within the last three years.
-
Knowledge of reimbursement regulations affecting Home Health arena preferred.

Computer experience.
-
Evidence of participation in ongoing CEU’s.
-
Participates in weekend / after hour home visits as required.

Current BLS Healthcare Provider Certification.
