Community NavigatorPosition Summary:
The Community Nurse Navigator collaborates closely in conjunction with community-based organizations, faith-based organizations, worksites and other healthcare providers in the community to promote population health and wellness. The Community Navigator also works with the Care Management Team to formulate and communicate transition/discharge plans that efficiently utilize health care services in a safe and cost-effective manner. In addition, the Community Navigator provides education and care coordination as needed for specific “at-risk” patient populations to reduce address to healthcare.
Qualifications:
A. Education:
Currently licensed as a Registered Nurse in the State of Texas. Bachelor of Science in Nursing is preferred.
B. Training and Experience:
Required:
· At least two years acute care experience
- Care Management, discharge planning preferred.
· Advanced knowledge of community resources
Preferred:
· Disease and/or quality management experience or community health experience
Unusual Physical Demands and Working Conditions:
A. Must be able to move about the hospital without major constraints.
B. Universal precautions are maintained at all times.
C. Must be able to work long hours on occasion and work well under tight deadlines.
