We are currently seeking an experienced:
LPN – PALLIATIVE CARE
Full Time (Remote)
JOB SUMMARY:
- Process incoming palliative patient referrals, to include using a variety of skills necessary for coordination of services.
- Assist Director of Palliative Care Services with Quality Assurance tasks and Payroll processing.
- Assist Nurse Practitioners in scheduling F2F visits for Hospice patients.
- Assist Nurse Practitioners with care coordination activities and referrals to other disciplines or services lines.
- Assist Nurse Practitioners with follow-up phone calls and visits related to advance directives and questions related to additional services.
- Assist Director of Palliative Care Services in day-to-day program tasks and tracking palliative referrals.
JOB REQUIREMENTS:
- Requires current LPN licensure within state of branch location and five years of clinical nursing experience, preferably in Home Care, Hospice or Skilled Nursing facility setting, to include the use Telehealth.
- Successfully completed nursing education from an approved school of nursing
- Must be able to independently organize work, communicate patient needs, and provide patient education and coordinate patient visit schedule(s).
- Must accept travel and working conditions as assigned.
- Must possess effective communication skills (oral and written), as well as, CPR certification.
- Minimum of twelve hours of continuing education in a twelve-month period
