Summary/Objective
The Social Worker shares the vision of the Little Siters of the Poor which is to contribute to culture of the life of residents, staff and families at Holy Family Residence. The Social Worker works collaboratively with the Administrator, Mother Superior, Little Sisters, Management Staff, Admissions Committee, and all other staff to serve as an advocate for the neediest elderly to ensure each resident is valued and treated with dignity and respect. The Director of Social Services serves as an advocate for residents and educator regarding residents’ rights, supporting resident’s choices and promoting the Home’s philosophy of person-directed care. These services are consistent with the Litte Sisters of the Poor’s standards and philosophy and are in compliance with all regulations, codes, policies and procedures that govern the Home. This role requires a licensed professional who can effectively assess client needs, coordinate services, and ensure smooth admissions processes.
Key Responsibilities
- Conduct comprehensive assessments of clients to determine eligibility and needs during the admissions process.
- Coordinate and facilitate client admissions, ensuring all documentation and requirements are completed accurately and timely.
- Develop individualized care plans in collaboration with clients, families, and interdisciplinary teams.
- Provide ongoing support and counseling to clients and their families.
- Maintain accurate and confidential client records in compliance with regulatory standards.
- Collaborate with healthcare providers, community resources, and other stakeholders to ensure continuity of care.
- Demonstrate dependability by consistently meeting deadlines and maintaining regular attendance.
Qualifications
- Licensed Social Worker (LSW, LGSW, or equivalent) in the state of Minnesota.
- Experience in the Long Term Care Setting
- Experience with client admissions processes preferred.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Demonstrated dependability and professionalism in previous roles.
