Position Overview
AMCR is seeking a compassionate and motivated Nurse Practitioner to join our growing endocrinology team in Escondido, CA. This role focuses on providing comprehensive outpatient care for patients with diabetes, obesity, and other metabolic conditions.
The ideal candidate is passionate about patient education, chronic disease management, and collaborative care delivery. This is an excellent opportunity for a Nurse Practitioner with endocrinology, primary care, or family medicine experience who is interested in building expertise in metabolic health and diabetes technology.
What Youll Be Doing
Clinical Care & Patient Management
- Conduct comprehensive patient assessments, histories, and physical exams
- Develop and manage individualized treatment plans for diabetes and metabolic conditions
- Prescribe and manage medications, including insulin and GLP-1 therapies
- Monitor patient progress using CGMs, insulin pumps, EMR data, and other clinical tools
- Manage and grow an established patient panel within a strong referral network
- Support patients with obesity, thyroid disorders, and other endocrine conditions
Patient Education & Support
- Educate patients and families on diabetes management, nutrition, lifestyle modifications, and self-monitoring
- Assist patients with Continuous Glucose Monitors (CGMs) and insulin pump initiation and management
- Encourage long-term adherence to treatment plans and self-management strategies
Collaborative Care
- Work closely with endocrinologists, diabetes educators, dietitians, and care coordinators
- Participate in multidisciplinary case discussions and care planning
- Coordinate with virtual care and remote patient monitoring teams when appropriate
Documentation & Quality
- Maintain accurate and compliant documentation within the EMR
- Participate in quality improvement initiatives and patient outcome tracking
- Stay current on advancements in diabetes and metabolic care
Qualifications
- Active Nurse Practitioner license with prescriptive authority in California (or ability to obtain)
- 6–12+ months of experience caring for patients with diabetes in endocrinology, primary care, family medicine, or internal medicine settings
- Strong interest in endocrinology, diabetes care, and metabolic health
- Excellent communication and patient relationship skills
- Comfortable working in a collaborative, fast-paced outpatient environment
- Experience with CGMs, insulin pumps, remote patient monitoring, or virtual care is a plus
