Licensed Practical Nurse - Team Leader (LPN)
Lake Regional Health System is looking for an outgoing, professional LPN - Team Lead to join our Palliative/Chronic Care team. With our state-of-the-art hospital and network of clinics, therapy sites, and pharmacies, Lake Regional offers a wide range of employment opportunities in our community.
Key Responsibilities:
- Works collaboratively with the Chronic Care Management (CCM) staff and manages a caseload of patients. Documents and codes are encountered appropriately.
- Providing education/teaching and identifying health care goals based on patient preferences for in-patients, cancer center, and clinic patients enrolled in CCM with chronic conditions.
- Assists with the completion of advance directives and TPOPP forms.
- Call patients monthly to assess, educate, answer questions, support, and encourage them to meet their health care goals.
- Proactively identifies deficiencies in patient knowledge of disease and management of disease symptoms and medications in an effort to improve patient disease management and to help decrease patient emergency room visits and hospital admissions.
- Consults social services, dietitians, chaplains and other services as dictated by patient needs.
- Recruits/enrolls patients in CCM when the patient meets the criteria.
- Assist with community education regarding palliative care, chronic condition management, hospice, and advance directives.
- Helps coordinate care of patients with the physician.
- Maintains Competencies, Certifications, and Licensure.
- Utilizes effective time management skills.
- Responds quickly and accurately to changes in condition or response to treatment.
- Ability to work with other members of the healthcare team to meet patient needs.
- Ability to interact positively and to communicate well with patients, families, coworkers, employees, supervisors, and other hospital employees.
- Cross-trained to provide post-anesthesia care. Responds quickly and accurately to changes in condition or response to treatment.
- Maintains regulatory agency requirements, nursing and hospital policies, procedures, and standards.
Qualifications:
- Current Missouri license and BLS required (may be provided by Lake Regional)
- An annual influenza vaccine is required.
