Home Hemodialysis Dialysis Registered Nurse
Coordinates and provides specialized nursing care to patients receiving home hemodialysis, with primary responsibility for patient training, ongoing assessment, education, and support for home-based dialysis therapies. Serves as a clinical resource for home hemodialysis patients and the interdisciplinary care team while ensuring compliance with CMS, ESRD Conditions for Coverage, and organizational standards.

MINIMUM EDUCATION REQUIRED
|
- Associate Degree: Nursing
|
MINIMUM EXPERIENCE REQUIRED
|
- Minimum of 12 months RN experience with minimum 6 months home dialysis experience required; dialysis, nephrology, home health, or chronic disease management experience preferred.
|
LICENSES / CERTIFICATIONS REQUIRED
|
- Current BLS and/or CPR certification from the American Heart Association or American Red Cross or the ability to obtain certification within 90 days of hire.
- Licensure to be eligible to practice as a Registered Nurse or Graduate Nurse in the State of Wisconsin.

KNOWLEDGE, SKILLS & ABILITIES REQUIRED
|
- Knowledge of CMS ESRD Conditions for Coverage related to home dialysis.
- Ability to read, write, hear, speak and comprehend the English language.
- The physical, technical and cognitive ability as well as the manual dexterity, to use patient care equipment.
- Basic computer skills.

PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES
|
- Responsible for the day-to-day operation of the home hemodialysis training facility.
- Provide patient, family, and support person, or persons, training for home-based dialysis.
- Complete regular assessments to monitor patients’ progress in managing home-based dialysis.
- Provides on-call services to patients during off hours and as regularly scheduled.
- Provides training for self-cannulation techniques, (AVF, AVG, Buttonhole, and catheter access) assessment, and care for access.
- Provides training to manage emergencies (emergency disconnect, blood leaks, air embolism risks, power failure, and emergency evacuation).
- Provides training and troubleshooting alarms on dialysis machine and equipment.
- Coordinates source water quality and required microbial testing of water source, dialysis machine, and reverse osmosis machine.
- Ensure safe storage, use and management of supplies.
- Ensures home hemodialysis documentation meet CMS requirements.
- Monitors and tracks home hemodialysis quality indicators and reports findings in QAPI meetings.
- Coordinates and monitors home medication therapies.
- Provides training on blood samples, labeling, use of a centrifuge, and handling/transporting of specimens.
Salary Range: $33.00 to $51.50/hr (based on experience)
