JOB SUMMARY
Compensation – $300,000 annual base salary
Job Type: Full-Time (W-2) or Part-time (W-2)
This Physiatrist (PM&R Physician) role focuses on delivering high-quality rehabilitation care within a structured post-acute care program. The physician will work collaboratively with therapy teams, nursing staff, and interdisciplinary care partners to improve functional outcomes, optimize pain management, reduce avoidable hospitalizations, and support safe patient discharges and care transitions.
The position offers a predictable schedule, strong clinical support, and opportunities for professional growth across multiple rehabilitation care settings.
ROLE AND RESPONSIBILITIES
Below are the roles and responsibilities expected of the PM&R physician:
• Provide comprehensive rehabilitation care to post-acute and long-term care patients.
• Treat patients recovering from stroke, debility and deconditioning, fractures, orthopedic procedures, joint replacements, spinal disorders, chronic musculoskeletal pain, and neurologic mobility impairments.
• Focus on improving patient mobility, functional independence, pain control, and overall recovery outcomes.
• Participate in a structured Skilled Nursing Facility (SNF) physiatry program with defined visit cadence and established clinical pathways.
• Collaborate closely with physical therapy, occupational therapy, nursing teams, and interdisciplinary care staff to support coordinated treatment plans.
• Contribute to efforts that reduce avoidable hospitalizations and promote safe care transitions and discharge planning.
• Engage in case conferences, educational sessions, and interdisciplinary learning opportunities.
• Work alongside an experienced team of physiatrists to ensure high-quality patient care and clinical best practices.
• Optionally participate in additional clinical settings, including inpatient rehabilitation facilities (IRF), acute hospital consult services, and outpatient clinics.
• Maintain accurate and timely clinical documentation and adhere to organizational and regulatory standards.
• Perform other duties as assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS
• Board Certified or Board Eligible in Physical Medicine & Rehabilitation (PM&R)
• Active Illinois medical license or eligibility to obtain licensure
• Experience in SNF or post-acute care preferred but not required
• Ability to work independently while functioning effectively within a team-based care model
• Strong clinical decision-making, communication, and patient care management skills
• Commitment to improving functional outcomes and patient quality of life
