Marina Harbor Detox, is a proud member of the Quadrant Health Group, is seeking a compassionate and detail-oriented Licensed Vocational Nurse (LVN) to join our night shift team in San Francisco, CA. This role is critical in supporting clients through medical detox and residential substance use treatment while ensuring safe, compliant, and high-quality careThis position reports to the Program Director.
What You'll Do:
Client Care & Health Monitoring
Conduct nursing assessments upon client admission, including completion of intake documentation and required consents.
Perform ongoing health assessments, including vital signs, withdrawal monitoring, and behavioral observations.
Conduct and document COWS and CIWA assessments per detox protocols.
Complete patient safety rounds every 30 minutes (or per facility protocol).
Monitor and document changes in client condition and promptly notify providers as necessary.
Provide detailed shift reports to ensure continuity of care.
Medication Administration & Documentation
Administer medications per physician orders and California nursing regulations.
Oversee and document medication self-administration when applicable.
Maintain accurate medication logs, including controlled substance counts.
Transcribe physician telephone/verbal orders per policy.
Communicate with pharmacies and prescribing providers regarding medication orders and clarifications.
Ensure proper medication storage, inventory control, and destruction in compliance with state regulations.
Testing & Clinical Support
Collect and supervise urinalysis (UA) and breath alcohol (BAC) testing.
Perform TB testing and required health screenings.
Assist with medical emergencies and crisis intervention as needed.
Maintain infection control standards and ensure medical stations remain organized and compliant.
Compliance & Collaboration
Maintain compliance with:
California Board of Vocational Nursing and Psychiatric Technicians (BVNPT) regulations
California Department of Health Care Services (DHCS) substance use disorder treatment standards
HIPAA and 42 CFR Part 2 confidentiality laws
Collaborate with clinical and leadership teams to ensure coordinated, high-quality care.
Participate in audits, inspections, quality improvement initiatives, and staff trainings.
Required Qualifications
Active, unencumbered California LVN license.
Active RADT (Registered Alcohol Drug Technician) certification through a DHCS-approved certifying organization (required).
Current CPR/BLS certification.
Minimum 6 months of experience in detox, behavioral health, residential treatment, or related healthcare setting (preferred but strongly encouraged).
Experience with medication administration and controlled substance monitoring.
Ability to conduct and document COWS and CIWA assessments (preferred).
Ability to work overnight (graveyard) shifts.
Successful completion of background clearance, drug screening, and required health/TB screening.
Skills & Competencies
Strong knowledge of detox and withdrawal management protocols.
Understanding of California SUD treatment regulations.
High attention to detail with strong documentation skills.
Ability to work independently during overnight shifts.
Professional communication skills and strong ethical standards.
Ability to remain calm and effective in crisis situations.
