Role Summary
Provide one-on-one skilled nursing care to patients in their homes.
Key Duties
In-home nursing assessments
Wound care, injections, catheter care, meds
Monitor condition & report changes
Patient/caregiver education
EHR documentation
Team collaboration & safety compliance
Requirements
Active LPN/LVN license
2+ years nursing experience (home health preferred)
Strong communication & assessment skills
EHR proficiency
Valid driver’s license & reliable transportation
