The SJ/C Physician Network Clinical Quality Coordinator coordinates the activities related to patient quality reporting and provides evidence based nursing assessments to patients attributed to SJ/C Physician Network. The Coordinator's primary function is to improve population health and patient specific positive clinical outcomes via performance improvement initiatives as assigned by manager including direct patient outreach via tele communication when appropriate. This position is responsible for regulatory compliance as needed. Serves as a positive and productive member of a multidisciplinary care coordination team. Appropriately communicates with management, physicians and staff. Uses strategic planning processes in collaboration with Administrative and Physician Leadership of Physician Network. Supports staff performance (attendance, tardiness, productivity) related to clinical outcomes for patients of SJ/C Physician Network.

Bachelors of Nursing - Required

3-5 Years Clinical experience (not limited to experience in long term care, community health, and/or home health) - Required

Operating position serving in reportable clinical outcomes for regulatory or accrediting agency - Preferred

Registered Nurse License with State of Practice - Required

Basic Life Support (BLS) - Required

Demonstrates the ability to provide patient care interventions. Effectively uses the nursing/patient care process to care for patients. Supports patient satisfaction.

Participates in operational and capital budgeting with manager. Eliminates waste. Uses resources wisely.

Coordinates and aligns quality strategic plan with operations strategic plan including scheduling, providing tele-health patient care, and regularly scheduled communication with quality. Promotes accountability for patient interventions. Promotes physician and quality agenda satisfaction.

Promotes staff development of generalist skills with expert in assigned service. Assists in development of mentoring and precepting relationships. Assesses staff for learning needs. Coordinates staff in-service and education. Educates on best practices. Maintains a learning environment. Supports employee satisfaction through positive patient care.
