Position Summary
The Patient Navigator will work collaboratively with patients and all the team members to navigate the health care system so that care can be delivered effectively based on the individual patient’s needs and assets. The Patient Navigator will be a point of contact for the clinic and will work with National Jewish Health , referring entities, CO-MTN (Colorado Multidisciplinary Translation Network) and patients to seamlessly coordinate care. The Patient Navigator will work with the patients, assist in connecting patients with available resources, and support the patient during clinic visits.
Essential Duties
- Obtains, organizes and maintains patient information, clinical forms, diagnostic results, and other information to ensure that necessary information is available at the time of appointment.
- Coordinates activities and flow of work for the assigned function or area (LongCovid Clinic).
- Serves as a resource to staff regarding departmental/area policies, procedures and services.
- Acts as one of the first points of contact for the clinic. Prepares and completes all required documentation in a thorough and accurate manner.
- Works collaboratively with National Jewish Health staff and referring clinic’s staff to provide seamless care and care transitions. Facilitate referral process and exchange of patient related medical records, assist in obtaining “release of information” forms.
- Orients patients to clinic upon visit.
- Participates in pre-clinic huddles to prepare for clinics and facilitate communication and coordination between health care team members.
- Assists with efficiency in clinic flow and patient visits. Anticipates and plans for potential problems or issues associated with upcoming visits.
- Maintains supplies in exam rooms and laboratory supplies to ensure uninterrupted service.
- Performs all assigned diagnostic procedures in accordance with ATS and departmental guidelines. Document services and other care related items.
- Works with outside vendors and patients as needed to address equipment and customer service issues. Assists with providing proper documentation to DME companies to facilitate the patients DME needs.
- Utilizes follow-up system to assure care follow-up and coordination is accomplished.
- Collaborates with multidisciplinary teams to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures.
- Facilitates the development, implementation and evaluation of program or area services and initiatives.
- Facilitates the development of practice standards and measurable outcomes based on performance evaluation and research.
- Performs competencies (including age-specific competencies) as identified through the departmental competency program.
- Adheres to state and federal regulations governing the disclosure of private health information.
- Participates in the development, data collection, and analysis of team-based quality improvement projects.
- Supports projects (research and quality improvement projects) that will contribute to improving patient-care, staff development and the mission of the institution.

Minimum Qualifications
- Education: High school graduate or equivalent preferred. Graduate of an accredited Medical Assistant program or other medical program that meets the criteria of a medical assistant program or successful completion of an approved nurse aide training program/Certified Nursing Assistant Program.
- Work Experience: 1 year of experience as a Medical Assistant or Certified Nursing Assistant required
- Special Training, Certification or Licensure: CPR/BLS required; Medical Assistant or CNA certification required
Salary Range: $25.04 - $33.11
