Position Summary:
The Specialty Pharmacist is responsible for patient care related to the dispensing of specialty medications and fluids ordered by a prescriber, as well as to provide patient education and ongoing clinical evaluation with relation to specialty medications and disease state management and to verify and assist physicians with medication selection, dosage calculations as well as maintaining accurate dispensing records and patient records for each patient that is cared for by the pharmacy. This position carries the responsibility to ensure accurate and quality pharmacy and customer service is provided in a professional manner in accordance with current federal, state, local standards, guidelines, and regulations.
Qualifications:
Education:
- Bachelor of Science Degree in Pharmacy or PharmD from an accredited College of Pharmacy required.
- Board certification in relevant specialty preferred (BCACP, BCOP, etc.).
Experience
- Previous experience in hospital pharmacy preferred.
Licensure/Certification/Registration
- Currently licensed by the State of South Carolina Board of Pharmacy in good standing required.
- Advanced Cardiovascular Life Support (ACLS) certification must be obtained within six (6) months of employment.
Duties & Responsibilities:
- Ensure the completion prescription verification as well as order verification before the end of each business day.
- Ensure the accuracy of all packaging, delivery and/or shipping of medications, supplies and teaching materials to the patient’s home according to the guidelines established by CMC Specialty Pharmacy.
- Communicate pertinent information to all applicable physicians, providers, customer service, authorizations and marketing to keep them informed of current patient orders and/or changes in the patient’s care, if required
- Contact and confer with physicians regarding questions or irregularities on prescriptions.
- Perform and document clinical monitoring.
- Assist in managing and maintaining complete and accurate patient records and operating records.
- Assist in managing referrals and help to troubleshoot case management when needed.
- Maintain direct and ongoing communication with the patient, patient’s caregiver(s) and other applicable professionals involved in any aspect of the patient care if applicable to include optimal cost-effective drug therapy.
- Supervise certified pharmacy technicians, Prescription Access Liaisons (PALs), and Prior Authorization Specialists in all routine performance of their daily duties.
- Provide and oversee the accuracy of all patient education, information, counseling, and pharmacy care furnished to CMC Specialty Pharmacy patients.
- Monitor drug therapy for contraindications, interactions, allergies, and appropriateness.
- Assist in maintaining operating efficiencies and accreditation standards within the department.
- Have a good understanding and working knowledge of all the Specialty Pharmacy policies and procedures, accreditation requirements and HIPAA guidelines.
- Maintain inventory and records of controlled drugs and other drugs as required by law or CMC.
- Participate in all inventory control programs such as ordering, inventory, stocking, pricing, quantities and monitoring of usage as required by CMC Specialty Pharmacy.
- Attend and participate in programs, committees, meetings, and other functions as required by the facility or pharmacy.
- Participate in pharmacy orientation programs, staff meetings and other in-services or educational curriculums.
- Assist in compiling necessary reports and/or audit information as requested.
- Work cooperatively with other staff members to promote a positive and team-oriented work environment.
- Participate in the Performance Improvement Program.
- Act as a disease state subject matter expert by maintaining current knowledge of drug therapies, drug reactions, and interactions by reading professional literature, attending seminars and utilizing professional affiliations in assigned disease state.
- Comply with CMC Ethical Business Conduct policy and Compliance Program.
- Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists.
- Complete required training, as assigned, within the established timeframes.
- Maintain regular attendance in accordance with established policies.
- Perform other job-related duties as assigned.
