Our two campuses in Methuen and Haverhill have a total of 320 beds, and we are a full-service, acute care hospital offering comprehensive inpatient medical/surgical units, outpatient care, and 24/7 emergency services. We have three behavioral health units: cardiac telemetry, and critical care service lines. Our Haverhill campus offers all private rooms, and our Methuen campus offers a new state-of-the-art catheterization suite and Joint Center of Excellence, named as a Blue Cross Blue Shield Blue Distinction+ Center for Knee and Hip Replacement.
Job Description:
- Provides thorough, timely, and accurate documentation of all pertinent data, therapeutic interventions and patient responses including assessment, care plan, patient goals, and interventions according to established standards.
- Provides timely and thorough transfer of nursing care on assigned patients to other health care team members to ensure integration of services and continuity of patient care.
- Assumes accountability for remaining current regarding standards of care established by the profession, regulatory and accreditation bodies and hospital policies and procedures.
- Track, follow and guide all new breast cancer patients from the time of diagnosis until they are handed off to Medical Oncology and Radiation Oncology
- Educate patients with respect to surgical procedures, breast imaging, needle localizations, sentinel node injections, drains, postop care and expectations after surgery.
- Follow and track all patients having US and Stereo biopsies to confirm pathology, diagnosis and concordance and having results available timely to the ordering physician.
- For benign results, plan follow-up, atypical or other plan and review next steps with patient and physician.
- Track all missing mammograms, US or discs from outside institutions.
- Follow all Breast MRIs ordered for pre-authorization with front desk staff, dates and times for the imaging, and checking results timely for ordering surgeon with recommendations to patients.
- During intake, identify high risk patients with genetic defects that need close follow-up. Subsequently meet and discuss follow-up plan of annual mammogram.
- Maintain/update risk assessment tool incorporated in mammography intake form.
- Review patient’s needs for social work, mental health support, and community support.
- Review patient’s needs for referrals for lymphedema physical therapy and treatment.
- First responder to telephone call from patient after the front desk staff fields a phone call with a question they cannot answer. Understand when to contact physician for further discussion and follow-up.
- Ensure every patient gets a phone call back by the end of the workday.
- Provides a wellness check on patient within 48 hours of biopsy.
- Closely tracks the BIRAD 4 and 5 groups to ensure patient follow-up.
- Provides a resource for information on disease, the advantages/disadvantages of treatment options.
- Works independently with own initiative and follow-up of patients and their care plan, but also collaboratively with all staff at the Breast Care Center for support.
- Maintains established processes and oversees current software application for the navigation of breast health patients and to expand the navigation program to include other cancer specific sites.
- Collaborates with oncologists, radiologists, and surgeons to establish the Breast Program Leadership team (formerly the BCC Committee)
Qualifications:
- Graduate of and accredited school of Nursing
- Current Nursing registration in Massachusetts
- BSL required.
- A certified breast health navigator or eligible for certification at the time of hire preferred.
- Demonstrated clinical competence in the specialty area of nursing practice preferred.
