The Care Coordination Director is a responsible for executing comprehensive care coordination strategies across the hospital. This role provides oversight and direction for the Case Management Program, ensuring that patients receive timely, appropriate, and patient-centered care throughout their hospital stay and beyond.
The Director also leads Utilization Management efforts to ensure efficient use of hospital resources, compliance with payer requirements, and alignment with clinical best practices. They also oversee Discharge Planning, facilitating safe and effective transitions of care to reduce readmissions and improve patient outcomes.
A key responsibility is to establish and maintain a system of care coordination and integration at both the individual patient and system levels, fostering collaboration among clinical areas, physicians, and post-acute care providers. Additionally, the Director is responsible for managing financial, clinical, and resource utilization outcomes using data-driven insights to improve performance, reduce costs, and support the hospital’s mission of delivering high-quality, value-based care.
This position requires 3 years' experience in a supervisory leadership position, preferably with Case Management or Utilization Management responsibilities.
