Position Summary:
The Discharge RN (DRN) will collaborate with interdisciplinary teams to coordinate resources and ensure that patient discharges meet established standards of care. The DRN is responsible for providing patient education, facilitating a smooth transition, and promoting quality outcomes. The Discharge Nurse will be expected to provide direct patient care to an assigned unit as determined by the AVP or the CNO
Qualifications:
Education:
- Associate degree (ASN) in Nursing required.
- Bachelors’ degree (BSN) in Nursing preferred.
Experience
- Three (3) years acute care experience preferred.
- Knowledge of nursing theory and practice required.
Licensure/Certification/Registration
- Current South Carolina (or compact state) licensure as RN (SCLLR) in good standing required.
- Basic Life Support (BLS) certification required.
- ACLS certification preferred.
Duties & Responsibilities:
- Ensure Completion of Discharge Requirements: The DRN would verify that all necessary discharge requirements are completed by the provider before initiating discharge paperwork, minimizing any delays.
- Medication Reconciliation: They would ensure accurate medication reconciliation, confirm patient preferred pharmacy and/or meds to bed.
- Follow-up Appointments: The DRN would be responsible for ensuring that patients have all necessary follow-up appointments scheduled prior to leaving the facility.
- Equipment and Home Services: They would communicate with case management and the primary RN to ensure the arrangement of needed equipment and/or home services have been made before discharge.
- Diagnosis Bundles: The DRN would ensure that all patient-specific diagnosis bundles are completed, working collaboratively with the nursing staff. (Discharge RED, etc.)
- Patient and Caregiver Education: The role would include providing comprehensive discharge education for patients and caregivers, addressing any questions they may have to promote understanding and compliance.
- Participation in Huddles: The DRN would participate in daily huddles to prepare for upcoming discharges, contributing to better communication and planning.
Standards of Performance:
- Monitoring and Quality Metrics: The DRN would monitor discharge statistics and maintain quality metrics to provide evidence of the positive impact of their work on patient outcomes.
