We are seeking a dedicated and compassionate Team Lead for our Intensive In Home program, located within our Administrative department. This full-time, hourly position is an excellent opportunity for a strong leader who is passionate about making a difference in the lives of individuals and families. As the Team Lead, you will oversee care delivery, provide guidance to team members, and ensure high-quality outcomes for our clients. Fluency in English is required for this role.
Essential Duties and Responsibilities:
- Supervise and support a team of service providers within the Intensive In Home program to ensure optimal client outcomes.
- Develop and implement care plans tailored to meet the needs of clients and ensure program compliance.
- Facilitate regular team meetings to foster communication, collaboration, and professional growth.
- Coordinate with other departments and external agencies to provide comprehensive care and services.
- Monitor program performance metrics and provide recommendations for continuous improvement.
- Ensure accurate completion of all administrative tasks, including documentation and reporting.
- Serve as a point of contact for resolving escalated issues, providing conflict resolution when necessary.
Qualifications
- Proven experience in a leadership role within a healthcare or social services environment.
- Strong understanding of Intensive In Home services and client care protocols.
- Excellent communication skills, with fluency in English being essential.
- Problem-solving and decision-making abilities to address issues effectively.
- Demonstrated ability to manage and motivate diverse teams.
- Familiarity with administrative and reporting requirements in a care setting.
