Position: Experienced Home Health Licensed Practical/Vocational Nurse (LPN/LVN)
Responsibilities:
-
Conduct nursing assessments of patients in their homes to develop individualized care plans.
-
Provide skilled nursing procedures such as wound care, medication administration, injections, catheter care, and monitoring vital signs.

Monitor and report patient status, changes, and responses to treatment to supervising providers.
-
Educate patients and caregivers on medication, disease management, self-care, and prevention strategies.
-
Offer emotional support and guidance to patients and families with empathy and cultural sensitivity.
-
Collaborate with physicians, therapists, social workers, and aides to ensure coordinated, holistic care.
-
Document care accurately in electronic health records (EHR) according to policies and regulations.
-
Participate in care planning, team meetings, and quality improvement initiatives.

Follow infection control, safety, and emergency protocols to protect patients and staff.
Qualifications:
-
Graduate of an accredited practical/vocational nursing program with current LPN/LVN license in the state of employment.

2–3 years of nursing experience (home health preferred).

Strong clinical assessment, critical thinking, and nursing procedure skills.

Excellent communication and interpersonal skills for patient and caregiver education.

Knowledge of home health nursing practices and regulatory standards.

Ability to work independently, manage time, and prioritize effectively.

Proficiency with electronic health record (EHR) systems.

Valid driver’s license, reliable transportation, and willingness to travel within service area.
