Position Overview:
We are seeking an experienced Home Health Occupational Therapist to join our dynamic team. The ideal candidate will be dedicated to improving the quality of life for our patients by providing expert occupational therapy services tailored to individual needs. This role offers the opportunity to work autonomously while collaborating with interdisciplinary teams to ensure the highest standard of care.
Responsibilities:

Conduct comprehensive assessments of patients' physical and functional abilities.

Develop personalized treatment plans in accordance with physician recommendations and patients' goals.

Provide skilled occupational therapy interventions aimed at improving mobility, strength, balance, and overall function.

Educate patients and their caregivers on therapeutic exercises, safety precautions, and injury prevention strategies.

Monitor patients' progress and adjust treatment plans as necessary to optimize outcomes.

Document all clinical findings, interventions, and patient responses accurately and in a timely manner.

Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care.

Maintain compliance with regulatory standards, including HIPAA and Medicare guidelines.

Participate in ongoing professional development activities to enhance clinical skills and stay current with best practices.
Qualifications:
-
Graduate of an accredited occupational therapy program with a current license to practice in Washington, DC.

Minimum of 3 years of clinical experience as an occupational therapist, preferably in home health or skilled nursing.

Strong clinical skills in assessment, treatment planning, and therapeutic interventions for adults and older adults.

Ability to recognize and appropriately respond to medical emergencies.

Excellent interpersonal and communication skills for collaboration with patients, families, and interdisciplinary teams.

Proficiency in comprehensive occupational therapy evaluations and evidence-based treatment planning.
-
Experience monitoring progress, modifying treatment plans, and ensuring continuity of care.
-
Supervisory experience with COTAs preferred.
-
Strong time management and organizational skills; ability to work independently.

Proficiency with EMR systems (Kinnser WellSky or Homecare Homebase preferred).

Reliable transportation and valid driver’s license required.
-
BLS certification and other relevant certifications may be required.
-
Recent TB screen or chest x-ray.
