Position Overview:
We are seeking an experienced Home Health Occupational Therapist to join our dynamic team, serving patients in Fairfax, Loudoun, and Prince William Counties. The ideal candidate will be dedicated to improving the quality of life for our patients by providing expert occupational therapy services tailored to individual needs. This role offers the opportunity to work autonomously while collaborating with interdisciplinary teams to ensure the highest standard of care.
Responsibilities:

Conduct comprehensive assessments of patients' physical and functional abilities.

Develop personalized treatment plans in accordance with physician recommendations and patients' goals.

Provide skilled occupational therapy interventions aimed at improving mobility, strength, balance, and overall function.

Educate patients and caregivers on therapeutic exercises, safety precautions, and injury prevention strategies.

Monitor patients' progress and adjust treatment plans as necessary to optimize outcomes.

Document all clinical findings, interventions, and patient responses accurately and in a timely manner.

Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care and achieve desired outcomes.

Maintain compliance with regulatory standards, including HIPAA and Medicare guidelines.

Participate in ongoing professional development activities to enhance clinical skills and stay current with best practices in home health occupational therapy.
Qualifications:

Graduate of an accredited occupational therapy program with a current license to practice in the state of Virginia.

Minimum of 3 years of clinical experience as an Occupational Therapist, preferably in home health care or skilled nursing facilities.

Strong clinical reasoning and problem-solving skills, with the ability to adapt treatment plans based on individual patient needs.

Solid knowledge of assessment tools, treatment planning, and therapeutic interventions for adult and geriatric populations.

Ability to recognize medical emergencies and respond appropriately.

Excellent interpersonal and communication skills, with the ability to collaborate effectively with patients, families, and interdisciplinary team members.

Proficiency in conducting comprehensive occupational therapy evaluations and developing evidence-based treatment plans.

Experience supervising COTAs (Certified Occupational Therapy Assistants).

Strong documentation skills; EMR proficiency required (Kinnser Wellsky or Homecare Homebase preferred).

Reliable transportation and a valid driver’s license required.
